New Patients:

Please download both the Foxit Reader (for PC) or FormulatePro (for Mac) and New Patient Intake Form to your computer. You must install the Foxit Reader (PC) or FormulatePro (Mac) Program in order to process our form. These programs will allow you to complete our registration form, written as a "PDF" file, which can NOT be processed and saved using the commonly available and free Adobe Acrobat Reader program. Once you open the Form with the downloaded and installed Programs, you can follow the instructions to complete and securely send us the form.

ATTENTION: New Patients Scheduled To See Dr. Roseff

ALL new patients scheduled to see Dr. Roseff need to "pre-register" for their upcoming visit at RelayHealth, Dr. Roseff's secure online messaging center, by clicking HERE (Go to https://app.relayhealth.com/Patients/Registration.aspx if your browser does not allow you to click on the link).  Creating an online secure account will allow Dr. Roseff's office to communicate with their patients in a fashion that complies with privacy laws (HIPAA).  Once you become an established patient, your RelayHealth messaging account will also help Dr. Roseff and his staff to send "late-breaking news" to all patients, allow you to send brief questions to the office, and will permit Dr. Roseff and his designees to electronically prescribe supplements and medications directly to your pharmacy.  Let us know if you have any questions about this. 

Consent Forms

If you have been instructed to do so by Dr. Roseff's office, please print out the appropriate consent forms (those that pertain to you), review them, and then sign the forms where indicated.  Make sure you have discussed their contents with Dr. Roseff or his designee.  Of course, you should always bring any questions or concerns you may have to our attention.

• Please review and adhere to the following guidelines:
• If you're undergoing IVF with your OWN EGGS, use forms 1, 2, 3, and 4.
• If your IVF plan includes ICSI (injection of sperm into the egg), add form 5.
• If you were told that you are at a high risk for HYPERSTIMULATION SYNDROME (OHSS), such as in PCOS patients, add form 6.
• If you have extra embryos for cryopreservation (FREEZING), add form 7.
• If you will undergo a FROZEN EMBRYO TRANSFER (FET) cycle using embryos previously frozen, use forms 4 and 8 only.
• If you are an EGG DONOR, use forms 2, 3, 4, and 9.
• If you are the recipient of donor eggs, use forms 4 and 10.
• If you will undergo preimplantation genetic diagnosis (PGD) on your IVF cycle, use forms 1, 2, 3, 4, and 11.
